End of Life Care Worksheet

Woman Talking With Unhappy Senior Man Sitting In Kitchen At Home

If information is available, the DAIL Nurse Consultant will provide the field worker with information that may be helpful when speaking to the family. This will be provided under “additional information to provide to family.” Information may include if the individual is alert, orientation, activities of daily living (ADLs), oral intake, etc.

The field worker will contact the family, complete the worksheet and return it to the DAIL Nurse Consultant.

Please note you do not have to mark “family does not plan to pursue guardianship” if the family is in agreement. The section below the family contact box only needs to be completed if the family is not in agreement with the recommendations.

How to search ECE-TRIS for Professional Development courses

Follow along with Julie at the website for ECE-TRIS, Kentucky’s Unified Professional Development Registry for Licensed Programs, Registered Providers, Certified Homes, Head Start, and Public School programs.

Video Transcript

Hi. In this video, I’m going to give you a brief overview of how to use all the search functions and filters in ECE-TRIS. So I have gone to the ECE-TRIS website ece.trc.eky.edu, and then clicked on at the top, the calendar approved training tab. Now most people, when they search, probably come and put a title in and then scroll all the way down to the bottom and hit search.

But there are a lot of different filters you can use to find trainings that might be a great fit for you. So I want to talk through what these filters are.

We have location. If you’re looking for face to face trainings, you can pick the county you’re in and it will limit your search results to things that are taking place within your county.

You can choose the cost. If you don’t / can’t afford to pay for training and you need a free training or… It’s not something that your center is going to pay for, but you’re interested in more information on the topic. You can always check free, and look for trainings that way.

Age group and environment. You can limit it to trainings that are checked for infant toddler specifically, or for preschool specifically, particularly if you are doing maybe a CDA renewal and you have to take age appropriate trainings. That’s a way that you can search. There’s also for school age trainings if you work for maybe an after school program and that’s your primary age. If you are at a family child care home, that’s also a group that you can pick.

Delivery method face to face. Obviously, that’s you go somewhere face to face for training. There really are very few conference call trainings, but webinars would be, say, a Zoom call training. Independent study is normally… alot of times those are a packet that involves a reading and study on your own or web based on demand learning. So that’s like an online course that you can do in your own time.

Training types. There are some specific different trainings. For example, pediatric abusive head trauma. Connect the Dots. All of these are different specific trainings, CPR and first aid, CDA, etc.

Distance from you so you can say within a certain distance from a zip code. Again, that’s primarily going to be useful for face to face trainings.

You can search for training levels. If you get bored in trainings a lot and you’re taking level one and two trainings, you can filter for level three through five trainings and do some higher levels. In this course there was a self-assessment. If you had a 100% on all the level one things, maybe look for some higher level trainings.

The number of hours. Some trainings may be just a quick introduction for something in an hour. Some could be longer training that lets you really delve more in-depth about a topic. So you could use the training hours to filter that.

And regions. The regions, if you’re not sure of your region, ask your director. But these are the different regions across the states. And you can look for not just your county but in your region Trainings.

There are more search options hiding if you click. Show more search options here.

Target audience All staff, there’s also things specifically for Directors. Head Start trainings, trainer trainings, parents trainings, family childcare trainings.

And the core content subject areas which you took your self assessments based on those content subject areas. So you might want to choose to use one of those filters to find trainings in maybe the subject areas you’re strongest or weakest in, which would help you find trainings that will help you set your goals.

So you can pick any combination of things here to search for. So I’m not even going to pick a title or a keyword. You don’t have to. I’m going to say I want free trainings that are on demand or independent study and you say a three or four or five and let’s do professional development or program management and evaluation and then hit search.

And it only found one. Inclusive, equitable facilitation. That actually does sound interesting to me. Let’s see, let’s take off one of the subject areas and see if it only showed me things with both. Nope. All right. So I’m going to take off… I’m going to add level two back in and see what we come up with. Okay… let’s take the free off and I’m going to add back my second [competency area].

And now let’s see what we have.

Okay. There’s 97 trainings now that I didn’t say they had to be free.

So again, this is a great way for you to find trainings. You can turn on and off filters. So play around with the ECE-TRIS search and that will be helpful for you as you are searching for courses to put on your annual professional development plan.
