This course is designed to help case managers develop clear, accurate, and professional documentation skills that meet legal and service expectations. This course emphasizes the critical role case notes play in guardianship, not just as routine paperwork, but as a formal record that supports accountability, continuity of care, and informed decision-making.
Participants will learn how to write case notes that are timely, objective, and person-centered, using individuals’ names and focusing on their experiences, needs, and preferences. The course introduces practical strategies such as using the 5 W’s (who, what, when, where, and why) to ensure complete and meaningful documentation. It also highlights common documentation pitfalls and how to avoid them, including vague language, missing details, assumptions, and repeated notes.
In addition, learners will strengthen their professional writing by identifying and removing slang or overly casual language to ensure documentation is appropriate for legal and professional review. Through real-world examples, guided practice, and reflection activities, participants will build confidence in writing case notes that clearly “tell the story” of the person and their services.
By the end of this course, you will be able to produce documentation that is clear, respectful, detailed, and aligned with best practices in state guardianship.
