Developing an Evaluation Plan

For this Assignment you will be developing an Evaluation Plan based on the training session you have been working on throughout FET.

Tips

1) Use the training plan that you have been creating. This plan includes information about your Workplace Outcome that will be valuable in helping you determine what to include in your Evaluation Plan. A quick refresher: your workplace outcome is what you want a participant to be doing when they return to work, and will be a guide for your Level 3 and Level 4 questions.

2) When working on Level 3 and Level 4 you will want to avoid the use of yes/no questions.

Instructions

1) Complete the following work across the rows of the table for all levels.

  • Questions and Inquiry: What question are you asking/what are you wanting to find out.
  • Measures/Instruments: How will you collect the information. Don’t forget to add pre and post data collection, when applicable.
  • Data Collection: Document exactly when you will collect the data for each level.
  • Data Analysis: Define exactly how you will analyze/summarize what the data tells you.
  • Disseminate Results to: Identify who will receive the data once it has all been collected and analyzed.

2) Once complete please upload a copy using the button below for review. Please remember you may be asked to make edits to the assignment after submission in order for the grading to be final. This course allows you to continue working while assignments are being graded, but assignments must be graded at a passing score in order to complete the course.

Grading Rubric – use this rubric to help understand how the assignment will be graded. A word document with grading rubric can also be found in FET 1 under Handouts.
