PATH Steps

So how to do you create a PATH?

  1. Locate the North Star
    The PATH process starts with the question of “what are our deepest hopes and dreams for” the person to create the North Star which will guide their work together.   
  1. Create Vision of a Positive Possible Future
    Then the team generates a vision of a positive and possible future for a specific time frame, e.g., six months or one year, in relationship to the dreams and hopes shared in the North Star. 
  1. Describe the Now
    Look at the “now” and compare where the person is with where they want to be in step 2.
  1. Invite Enrollment
    Identify the people who are needed and will support completion of the goal.
  1. Decide to Get Stronger
    What are the activities to sustain our efforts to create a positive and possible future.
  1. Agree to Next Steps
    What will be the next steps in our action plan.
  1. Organize the Next Month’s Work
    What will we do in the next month?
  1. Identify Bold Steps
    What can we envision that requires the dignity of risk balanced with health and safety concerns.
