Application Activity – Using PATH with Linda

Returning to Linda, she is 70, lives in Louisville, and receives services from the Supports for Community Living (SCL) Waiver for people with intellectual and developmental disabilities.  She lives in her own apartment in a building owned by a non-profit and is HUD subsidized.   She receives 50 hours/week in Personal Assistance services.  Linda uses a walker for stability and balance after falling in April of 2020 and fracturing her pelvis.  

PATH was used to guide her Person-Centered Service Plan.  One of the items identified in the “Positive and Possible” section of the planning was a trip to Disneyworld. 

  1. What is involved in planning for and going on such a trip?  
  2. What is necessary for Linda to make this trip safely and successfully? 
  3. Other thoughts or considerations?

Team Member Roles

Each of these steps can be mapped out in the PATH mural which creates a visual record of what is agreed to at the meeting and provide for accountability.

Member roles can be included as actions in the PATH mural in the sections titled “enrollment (e.g., guardian supports the planning process within the bounds of the guardianship court order), next month’s steps (Linda will save $200/money each month in the Guardianship Special Needs Trust, and First Steps (Buy a pedometer and commit to walking 6000 steps/day using her walker and staff support).
