10.Health Issues/Attendance (check the best description) 

• Attends at least 90% of school days. 
• Attends approximately 75% of school days; absences primarily due to health issues. 
• Attends approximately 50% or less of school days; absences primarily due to health issues. 

• Receives Homebound Instruction due to health issues. 
• Highly irregular attendance or homebound instruction due to issues other than health. 

Select the best description of the individual student health and attendance description.  

Students
with significant cognitive disabilities may often be absent from school
because of medical conditions.  Missing school is also missing
instruction. A student cannot be found eligible to participate in the
alternate assessment based on their attendance and health issues. 

This
information can be obtained through school attendance records and
through communicating with the parents about current health issues
and/or reasons for absences.
