Topic 5: Collaboration & Partnerships

What to Expect From Programs:

Individuals who participate in support services and programs will engage with a variety of professionals. These include case managers, service coordinators, therapists, educators, etc.

Collaboration across these programs ensures that individuals in a person-centered system are served with their needs, goals, and preferences in mind.

What is Collaboration?

Collaboration is the act of working together across roles and programs to support individuals in achieving their goals.

The key elements of collaboration include:

  • Shared Goals
  • Clear Communication
  • Respect for Roles
  • Empathy and Understanding

4 Pillars of Collaboration

1.) Shared Goals:

Everyone involved is working toward the same outcomes for the person being served, even if they have different roles or responsibilities.

2.) Communication

Open, honest, and timely communication helps avoid misunderstandings and ensures everyone stays informed and aligned.

3.) Respect for Roles

Each partner brings unique expertise and responsibilities, and recognizing these helps build trust and effective teamwork.

4.) Empathy and Understanding

Valuing each other’s perspectives and challenges fosters stronger relationships and better support for individuals.
