“A request for disbursement from the trust fund for an individual under guardianship may be made by any employee of the Division of Guardianship, the Fiduciary Branch, or the Benefits Branch.”

To make a request: 

1.) Guardianship staff will first complete the Guardianship Trust Pre-Screening Tool and submit it via email to their supervisor Director’s Office approval.

Benefits and Fiduciary staff should consult with their supervisor prior to submitting a request.

2.) After obtaining supervisor approval, staff will submit the request for disbursement through the Guardianship Trust Tracking System: https://sp13internal.chfsinet.ky.gov/sites/DAIL/gtf/SitePages/Home.aspx

If an employee does not have access to the Tracking System, they should contact their supervisor to request access.

3.) Once submitted, the request will proceed through the following chain of approval:

    Directors of the Division of Guardianship and the Division of Administration and Financial Management (or their designees)

4.) If the request is approved, Fiduciary Branch staff will enter a payment request for the trust fund disbursement in KYGFIS.
