Dr. Young enters room with Mr. and Mrs. Brown both present – Mrs. Brown is holding her infant.
Dr. Young
(smiling and maintaining strong eye contact) Good afternoon. Good to see you both, Mr. and Mrs. Brown. You certainly have a beautiful boy! Have you picked a name for him?
Mrs. Brown
Yes, we named him Craig Addison. The Craig after my father and the Addison from a family name on my husband’s side.
Dr. Young
Craig looks like he is doing well (gesturing to a contented, sleeping baby) in mom’s lap.
Mrs. Brown
He is doing fine, Dr. Young, though I am having a little trouble with nursing him.
? Decision Point: What would you say next?
A) I would immediately bring up my concern about the baby having Down syndrome. This is no time to delay voicing my suspicions.
Not the best choice: The mom has expressed a valid concern that you should first address.
B) I would first inquire a little more about the nursing before I broached my suspicion of the child having Down syndrome.
Good choice: You need to first listen to, and to address the mom’s concern about her baby’s breastfeeding.
