Scenario 1: Johnny Hertz

You receive notice from the administrator at Lakeside Personal Care Home, that Johnny Hertz broke his glasses and is having trouble seeing up close. He is upset because he can’t see his bingo card or play board games. Johnny is hesitant to go to the appointment, because he is afraid he will miss his preferred activities at the PCH. They have staff willing to take him and have made the appointment for next week.  They are seeking your assistance to get the necessary signed consent forms, gaining his cooperation to go to the optometrist, and arranging for payment for the replacement glasses.
